Proton beam therapy differs from standard photon-based radiation therapy in that the penetration of the heavy, charged protons is limited because of low energy, minimizing damage to surrounding structures and allowing dose escalation. Most published studies have not shown any substantial difference in cancer outcomes between IMRT and proton beam therapy and, interestingly, proton beam therapy has been associated with equivalent or even greater toxicity. Proton beam therapy is expensive, and in the absence of any obvious advantage in cancer control or toxicity, most radiation oncologists prefer tried and tested IMRT for prostate cancer.
